We are pleased that you have chosen our practice for your dental needs. We look forward to a long-term relationship with you and ask for your cooperation in adhering to the financial policies of our practice.
Payment is expected at the time services are rendered. We accept cash or check, as well as Visa, MasterCard, and Discover. We also offer a third party payment plan called CareCredit. CareCredit allows you to start your treatment immediately and spread your payments over time. There are interest-free as well as low interest options with the CareCredit plan.
Many offices charge their patients to file insurance claims, or have patients file their own claims. As a courtesy to you, we will file your dental insurance. Please keep us informed of any changes.
Charges not covered by your dental insurance policy are your responsibility. We will do all that we can to help get benefits extended to you, however if your insurance company does not pay within 90 days of the claim submission, you will be expected to pay us directly and wait for reimbursement from your insurance carrier. You may need to contact your insurance company to assure payment is made in a timely manner.
It is your responsibility to familiarize yourself with the details of your policy including deductibles, any waiting periods on covered treatment, preventative care frequencies, calendar versus insurance benefit year, and insurance year maximum benefits.
You will receive a statement of your account activity each month whether or not an insurance payment is pending. Delinquent accounts will be turned over to an outside collection agency. If you have any questions regarding your statement, please do not hesitate to call our office.
